Based on discussions, phone calls, and other feedback from members, we have added the ability for an employee to create a self reflection Year End Summary Note. Since the start of Folio, Notes have been used to document conversations between two people. Whether it was a Goals Meeting, a Year End Summary, or a Conversation, they have involved two people. Now with the self reflection Year End Summary Note, an employee can document their year end self reflection.
An added feature of the note (borrowed from the Custom Observation Note) is the ability for Folio School Admins to create custom Year End Note forms with any number of prompts the employee can respond to.
These forms can be chosen (along with the original, default Year End Summary form) by the author when creating a Year End Summary Note.
On the Customizations page (Manage-->Customizations), in the Custom Year End Self-Reflections section (down towards the bottom), type a name for your form in the Custom Forms drop-down and hit the enter/return key.
You can then create any number of “prompts” or questions that the employee can respond to.
The form content (prompts, template text) is saved with the Year End Summary Note, so you can edit or delete forms if you like and it won’t affect already published or drafted notes.
The user can select the custom form from a drop-down on the Year End Summary Note page.
Once the form is clicked on, the custom prompt(s) and response boxe(s) with the template text (if any) appear.
